To change or update your personal information in the Streams Church database...

The accuracy and security of the information requested is important to us. This system will allow us to identify you within the database.

If you are already in our database, then we will send you an e-mail to verify your identity and allow you to continue. This ensures that only you and our staff can view or change information associated with your profile in our database.

If you are a new user, then you will need to contact the Streams Church office at 623-363-7450 or fill out the visitor card information at a Sunday service.
